At Mara Weinberg Psychiatric NP PLLC DBA (“Mint Psychiatry”, "I" “we”, “our”, or “us”), we are committed to protecting your privacy and developing technology that gives you the most powerful and safe online experience. This Statement of Privacy applies to our website, www.mintpsych.com (the “Platform”) and our related services (together with the Platform, the “Services”) and governs data collection and usage. By using the Services, you consent to the data practices described in this statement.

Collection of your Personal Information

We collect personal information, including but not limited to; your e-mail address, name, home or work address or telephone number.

We may also collect, store, or process information when you:

Our Services also may automatically collect details about your computer hardware and software. This information is used for the operation of the service & to maintain the quality of the service. Protected Health Information

In using certain components of our Services, you may provide certain Protected Health Information (PHI) as defined under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (“HIPAA”) and substance use disorder treatment records as defined under Title 42 of the Code of Federal Regulations (“CFR”) Part 2: Confidentiality of Substance Use Disorder Patient Records (“42 CFR Part 2”). Our "Mint Psychiatry HIPAA Privacy Practices" is a separate document that governs how your health information may be used and disclosed by our practice. Please note that any medical information that you provide will be used and disclosed only in accordance with applicable state & federal law. 

 

Use of your Personal Information

We collect and use your personal information to operate the Platform and deliver the Services you have requested and/or additional services that could potentially be applicable to you. We may also use your information for the following purposes:

  • To fulfill any purpose for which you provide it;

  • To allow you to participate in any interactive features on our Services;

We may use cookies (small text files stored in your browser) and other techniques such as web beacons (small, clear picture files used to follow your online activities) (“Tracking Technologies”). Tracking Technologies collect information that tells us how you use our Site, web-related products and Services. The use of Tracking Technologies does not give us access to the rest of your computer.

This, in turn, helps us make our Services relevant to your interests and needs. We may use both first-party Tracking Technologies and third-party Tracking Technologies.

Our Platform is hosted and operated in the United States (“US”) and elsewhere in the world. By using this Site, you are consenting to the transfer of your personal information to the US. California’s “Shine the Light” law (Civil Code Section § 1798.83) permits users of our Website that are California residents to request certain information regarding our disclosure of Personal Information to third parties for their direct marketing purposes.
